No Suppression System Documentation
NFPA 75 requires documentation of the fire suppression system protecting IT equipment. Many facilities have systems installed but no ITM records showing the system has been inspected or tested within required intervals.

Improper Cable Penetration Seals
All cable penetrations through fire-rated walls, floors, and ceilings must be properly firestopped. This is one of the most common physical findings — and one that requires both physical remediation and documentation of the corrective action.

Clean agent systems (FM-200, Novec, CO2) require annual inspection and periodic weight verification. Facilities frequently have the system but can't produce dated inspection records showing agent quantity and pressure were verified.
